03How it works

Five steps. One practical partnership.

From the first conversation through to regular review, we keep the process clear and centred on the participant's goals.

1

Free initial conversation

Call, email or send an enquiry. We talk about the participant's goals, current supports and what help is being sought.

2

Confirm service fit and funding

We review whether the requested support fits RNC's current service scope, registration or eligibility, and the participant's funded plan.

3

Agree on the plan

We document goals, the supports to be delivered, frequency, location, responsibilities and review arrangements in a service agreement.

4

Take practical action

Nursing care, health education, vocational assessment, work preparation or employer liaison is delivered according to the agreed plan.

5

Review progress

We review what is working, document outcomes and adjust the approach when the participant's needs or goals change.

04Funding

Understanding NDIS funding for RNC services.

The right support category depends on the service being delivered.

NDIS employment supports can help participants build work skills, identify barriers, develop career plans, find suitable employment and maintain work. Community nursing is a separate class of support for disability-related nursing care. RNC will confirm the applicable support and funding arrangements before services commence.

Community Nursing CareNDIS registration group 0114. RNC service availability is subject to current provider registration or eligibility and the participant's funding arrangements.
Employment supportsEmployment funding may include assistance to build work skills, find or maintain employment, and employment-related assessment, counselling or advice. The exact support item depends on the participant's plan and the service delivered.

06Frequently asked questions

Good questions, clear answers.

Does RNC provide NDIS services across the Fraser Coast?

RNC is based in Toogoom and is developing NDIS service delivery for the Fraser Coast, with priority focus on community nursing and employment-related rehabilitation supports. Availability is confirmed individually based on service area, capacity and current provider registration or eligibility.

Do I need to already have a job to receive employment support?

No. Employment support can be relevant at different stages: exploring work, preparing for a first job, changing career direction, returning to work, or maintaining an existing role.

Can RNC check whether my plan can fund the support I need?

We can discuss the support you are seeking and help identify the relevant area of your plan. Before services commence, we confirm that the requested support is appropriate for RNC to deliver and that suitable funding arrangements are in place.

Is NDIS employment support the same as an Australian Government employment program?

No. NDIS employment supports are disability supports funded through a participant's NDIS plan. They can work alongside mainstream or government employment services where appropriate.

What kinds of nursing support can RNC provide?

Subject to the participant's needs, funding and RNC's current service availability, nursing support may include assessment, care planning, health monitoring, medication-related education, wound or skin care, and education for carers or support workers within nursing scope.

Can a support coordinator or health professional refer someone?

Yes. Referrers are welcome to use the enquiry form below. Please keep the initial website enquiry general and avoid sending detailed or highly sensitive medical information through the public form.

Your goals are a good place to start.
